On your first visit to Sierra Care Physicians, please bring:

Your insurance information
Any co-payments your insurance may require
A list of all medications you are currently taking
A face mask without an exhalation port

Please arrive ten minutes before your scheduled appointment to allow time for paperwork. This will assure that you will receive the best possible health care service and personal attention.

our billing procedures

We bill most insurances and Medicare. Our office fees reflect customary fees in our area and we strive to keep our costs at a reasonable level.

At the time of your visit we will need your proof of insurance and social security number. Insurance co-payments are paid at time of service.

missed appointments & other fees

In order that you not incur a missed appointment fee, we request a four hour advance notification for cancelled appointments. We are more than happy to reschedule you for another day.

Please bring any special forms such as camp or sports physicals, medical school forms, etc. If you do not arrive with the forms at your scheduled appointment time, an administration fee is charged for completion at a later date.
